Thank you for your interest in transferring the Elevate Limited Warranty covering the roof assembly on the above-referenced building. Under our current procedures and the conditions of the warranty, the warranty can be transferred upon receipt of this completed form and full payment of transfer fee.

Please note that all membrane warranties will be transferred along with the system warranty.

A transfer fee will be invoiced for each warranty. DO NOT SEND payment without first receiving an invoice and make sure to include the invoice number (Debit Memo Number) with your payment

Roof review was / was not performed by a currently licensed Elevate contractor within the past 90 days.
